Assess our client’s home and living situations and provide feedback.
Provide clarity on the ever-confusing care questions that elders and families have: What is a Continuing Care Retirement Community? What is an assisted living? What is independent living? How about home care? How do we pay for these various options? Will my long- term care insurance help? Hospice? Nursing home? HELP!
Help to identify resources that can assist with daily life.
Explain and facilitate family discussions on difficult topics such as DNRs (Do Not Resuscitate Orders) and Advanced Directives.
Help our clients make and short- and long-term care and life plans.
Accompany clients to important doctors’ appointments and help to communicate and facilitate the doctor/patient relationship so that the client is receiving the highest quality health care possible.
Visit on a regular basis to provide insight and feedback.
Coordinate all medical services.
Work with home and/or facility-based care providers to ensure continuity of care and implementation of client goals.
During a hospitalization or health crisis, serve as an educated advocate for the client and a bridge between the family and health care providers.
